NOTE: Electronic signatures are not allowed on the PDF version of Form I-9 and are only used in the following examples.  PDF Form I-9 requires original signatures. SOCIAL SECURITY CARDS

Please note that if an employee provides a Social Security Card as a List C document it must be UNRESTRICTED. A card that includes any of the following restrictions is not considered an acceptable document:

  • "NOT VALID FOR EMPLOYMENT"
  • "VALID FOR WORK ONLY WITH INS AUTHORIZATION"
  • "VALID FOR WORK ONLY WITH DHS AUTHORIZATION"
